Medication management, Health assessments, Care planning and coordination, Education on health conditions and treatment options, Monitoring chronic illnesses (diabetes, hypertension, etc.).

NOW (New Options Waiver) and COMP (Comprehensive Supports Waiver) programs provide services for individuals with intellectual and developmental disabilities so they can remain at home rather than in facilities.

Provides in-home and community-based services to help elderly and disabled individuals remain in their homes instead of moving to nursing facilities. Services include, Adult Daycare, Personal Care, Respite Care, and Home Delivered Meals.
